By utilizing Ascend’s services, the client acknowledges and agrees that the practice owner, organization, provider, or authorized representative remains solely responsible for:

• Maintaining active and compliant business registrations
• Maintaining current licensure, certifications, and provider eligibility
• Reviewing and approving all submitted information prior to submission
• Ensuring HIPAA compliance and internal security procedures
• Maintaining accurate and truthful records and documentation
• Responding to document requests and communications in a timely manner
• Retaining legal, tax, financial, insurance, and compliance professionals as necessary
• Making final business, financial, staffing, operational, and clinical decisions

Ascend shall not be held liable for delays, denials, rejections, interruptions, financial loss, or compliance issues resulting from incomplete information, inaccurate submissions, client delays, third-party payer decisions, provider eligibility matters, or responsibilities retained by the client.

Ascend Revenue Services, LLC. may provide educational information and consultations related to business protection strategies, life insurance, annuities, key person insurance, retirement planning concepts, and business continuity planning. Insurance-related products or services may be offered through properly licensed insurance professionals and are subject to underwriting, carrier approval, state regulations, and policy terms and conditions. Nothing contained within Ascend’s website, workshops, consultations, or communications should be interpreted as a guarantee of financial performance or future outcomes.
